Sheffield Churches Council for Community Care (SCCCC)

Sheffield Churches Council for Community Care works with partners and in the health and social care sector as well as with other voluntary organisations to provide a network of support to older and vulnerable people. Our services include the Good Neighbours Scheme where we offer the kind of help a good neighbour might give. Hospital Aftercare Service and A&E to Home Scheme - where we take an older person home after discharge from A&E and settle them into familiar surroundings.
